Quick answer

An international air ambulance moves a patient between countries with medical care throughout, by private ICU jet, by stretcher on an airline, or with a doctor or nurse in an airline seat. Safe Fly Aviation arranges all three to and from India, bed to bed, with licensed operators and airlines. A private ICU jet costs about ₹27 lakh to ₹45 lakh between India and the UAE and about ₹1.05 crore to ₹2.25 crore or more between India and the United States; a medical escort on a scheduled flight starts at about ₹2.5 lakh on the shortest routes. Each transfer is quoted in writing after the medical report is reviewed.

What an international air ambulance is

An international air ambulance service moves a patient across borders with medical care from the hospital bed at one end to the hospital bed at the other. The flight is only one part of it: the service also covers the ground ambulances, the medical team, the flight permits, the visas and the receiving hospital.

Critical or unstable patients fly on a private jet fitted as an intensive care unit. Stable patients can often travel on a scheduled airline, lying on a stretcher or sitting with a doctor or nurse beside them, which costs much less. The patient’s condition decides which is safe, and the time available and the price decide between them.

Every step from hospital bed to hospital bed is set out in the international bed-to-bed guide.

Three ways to travel between countries

Every international air ambulance enquiry is assessed against all three. The patient’s condition decides which are possible.

Private ICU air ambulance jet

For critical or unstable patients. The jet flies to the patient’s schedule, with a doctor, a nurse or paramedic and intensive care equipment on board.

Commercial airline stretcher

For stable patients who must lie flat. A stretcher is fitted in the airline cabin and a doctor or nurse travels alongside. The airline must give medical clearance first.

Medical escort in an airline seat

For stable patients who can sit upright. A doctor or nurse travels in the next seat with oxygen and medicines, often in business class on long flights.

Aircraft and medical equipment

The aircraft is matched to the distance, the patient’s condition and the airports at each end: a turboprop for short international routes, a long-range jet for the Gulf, Europe or Africa, and an ultra-long-range jet with a fuel stop for the Americas or Australia.

Photos show examples of each type. The aircraft for a transfer depends on the route and the operator available. Maximum range is the manufacturer’s figure; with a patient, a medical team and fuel reserves on board the practical range is shorter. Full figures: air ambulance aircraft.

On board an ICU flight

  • A stretcher, a ventilator, monitoring, infusion pumps, a defibrillator and oxygen.
  • A doctor and a nurse or paramedic, depending on the patient’s condition.
  • A neonatal transport incubator or an isolation unit can be arranged on some aircraft, depending on the case and the operator.
  • Helicopters are used for short legs, such as from a hospital to an airport; the international flight itself is fixed-wing.

How an international transfer works

  1. Call and medical report

Call or WhatsApp the 24/7 desk and send the medical report. Our team reviews the condition, the destination and the urgency with the treating doctor.

  2. Options and a written quote

We set out which ways to travel are medically suitable, the aircraft or airline, the medical team, and a written quote for each.

  3. Permits, visas and the bed

We obtain flight permits or the airline’s medical clearance, guide the visas, and confirm the bed at the receiving hospital.

  4. The flight

A ground ambulance takes the patient to the aircraft, and the medical team monitors and treats the patient throughout the flight. We keep the family informed.

  5. Arrival and handover

A second ground ambulance takes the patient to the receiving hospital, where the medical team hands over to the doctors there.

The air ambulance booking guide lists the documents needed at each stage.

Documents for an international transfer

Starting these early saves more time than any other step.

  • Medical report and fit-to-fly

A recent medical report and a fit-to-fly note from the treating doctor. Airlines also need their own medical form.

  • Passport and visa

Valid passports for the patient and companions. Patients coming to India for treatment need a Medical Visa; many nationalities can apply online for an e-Medical Visa.

  • Hospital acceptance

A letter or confirmation from the receiving hospital that it will admit the patient, with the treating specialist’s name.

  • Insurance or payment

A guarantee of payment from the insurer or assistance company, or payment arranged by the family, before departure.

International air ambulance: common questions

What is an international air ambulance service?

An international air ambulance service moves a patient between countries with medical care throughout, from the hospital bed at one end to the hospital bed at the other. Critical patients fly on a private jet fitted as an intensive care unit; stable patients can travel on a scheduled airline on a stretcher or with a doctor or nurse in the next seat.

How much does an international air ambulance cost?

A private ICU jet costs about ₹27 lakh to ₹45 lakh between India and the UAE and about ₹1.05 crore to ₹2.25 crore or more between India and the United States; a medical escort on a scheduled flight starts at about ₹2.5 lakh on the shortest routes. The price depends on the distance, the aircraft, the medical team and equipment, the permits and how soon the flight must leave. These budget ranges were reviewed in October 2026, and each transfer is quoted in writing after the medical report is reviewed.

What does bed-to-bed mean?

Bed-to-bed means one team arranges every step: the ground ambulance from the hospital, the flight with the medical team, the permits and visas, the ground ambulance at arrival and the handover at the receiving hospital.

When is a commercial flight safe instead of an air ambulance?

When the patient is stable and the airline’s medical department clears the patient. A patient who can sit upright can travel with a medical escort, and a patient who must lie flat can travel on an airline stretcher. Critical or unstable patients need a private ICU air ambulance.

How quickly can an international air ambulance depart?

International flights usually depart within 6 to 12 hours of the first enquiry, because flight permits have to be obtained first. Commercial stretcher and escort transfers take longer, usually a few days, because the airline must clear the patient.

Which countries do you cover?

Safe Fly Aviation arranges transfers between India and the Middle East, Europe and the UK, the Americas, Africa, Australia, Central Asia and South and South-East Asia, as well as within India.

Does insurance pay for an international air ambulance?

It depends on the policy. Many travel and international health policies cover medical evacuation or repatriation when a doctor confirms it is needed. Ask the insurer or assistance company early; we provide the written quote and case documents they ask for.

Does Safe Fly Aviation operate the aircraft?

No. Flights are flown by licensed air ambulance operators and scheduled airlines. Safe Fly Aviation arranges the transfer and gives families, insurers and case managers one point of contact from the first call to the hospital handover.
