An air ambulance is worth it when a critically ill patient needs a hospital that is many hours away by road, is somewhere roads cannot reach quickly, or has to cross a border. Within India a private ICU air ambulance costs about ₹4.5 lakh to ₹22.5 lakh. For a hospital a few hours away, an ICU road ambulance is often just as quick, and a stable patient can fly on a scheduled airline for much less. Some health insurance policies pay part of the cost.

When an air ambulance is the right call
An air ambulance is a flying intensive-care unit. Its value is distance: it takes a very ill patient to the right hospital in hours where a road journey would take a day.
The right hospital is far away
The patient needs intensive or specialist care that the local hospital cannot give, and the hospital that can is many hours away by road.
Roads cannot do it in time
Hill areas, islands and remote districts, where a road transfer would take too long or is not possible.
The patient cannot manage a long road journey
A ventilated or unstable patient for whom many hours in a road ambulance would be a risk in itself.
A transplant will not wait
A patient called for an organ transplant who has to reach the transplant hospital within hours.
The journey crosses a border
Bringing a patient home from abroad, or taking one to another country for treatment.
What an air ambulance is and how a transfer runs: what is an air ambulance?
When something else is better
Not every transfer needs a private aircraft. The treating doctor decides whether the patient is fit to travel and how; these are the usual patterns.
An air ambulance usually makes sense when
- The hospital that can treat the patient is many hours away by road
- The treating doctor says the patient needs intensive care on the way
- The patient is somewhere a road ambulance cannot reach quickly
- The transfer is to or from another country
Another option may be better when
- The hospital is a few hours away by road. An ICU road ambulance can leave at once and may arrive sooner.

Time: road against air
Families worry that arranging a flight wastes time. Over a long distance it saves a great deal. Over a short one it may not.

Add the time it takes to arrange the aircraft, typically 2 to 4 hours within India, and the ambulance journeys at both ends. On a route like Delhi to Jaipur that can cancel out the speed of the flight, which is why a road ICU ambulance is often the better choice there. On a route like Delhi to Guwahati nothing by road comes close.

Cost: three ways to fly a patient
A private aircraft is the most expensive way to move a patient by air and is not always the one that is needed. The patient’s condition decides which of these are possible.

What insurance pays
Some health insurance policies in India include air ambulance cover, usually up to a fixed limit. Star Health’s Comprehensive policy, for example, covers up to ₹2.5 lakh per hospitalisation and ₹5 lakh per policy period. Against a private air ambulance at ₹4.5–22.5 lakh within India, cover like that pays part of the bill, not all of it.
Insurers generally want the treating doctor’s recommendation that air transport is medically necessary and that road transport is not practical, and some ask for approval before the flight. Five questions to ask before you confirm.
Who will fly the aircraft?
Ask which operator will fly it, and whether that operator is approved by the Directorate General of Civil Aviation (DGCA).
Who is the medical team?
A doctor, nurse or paramedic matched to the patient’s condition. Ask about their critical-care experience.
What does the quote include?
Get it in writing and itemised: aircraft, medical team, equipment and the ground ambulances at both ends.
Have they flown this route?
International transfers need permits and, on airlines, medical clearance. Experience on the route saves time.
Can you reach them at any hour?
Call the number before you need it. Check reviews, and speak to someone who has used the service if you can.
