Before you fly to India
Three documents can delay a transfer from Africa. Start them as soon as the transfer is being considered.
Yellow fever certificate
India asks travellers arriving from countries with yellow fever risk, including Nigeria, Ghana, Kenya and Ethiopia, for a valid vaccination certificate. It becomes valid 10 days after vaccination. A traveller without one can be quarantined for up to 6 days on arrival, so tell us early if the patient or a companion has no certificate.
Medical visa
Patients need an Indian Medical Visa, with Medical Attendant visas for companions. At the time of writing, citizens of Kenya, Ghana and South Africa can apply online for an e-Medical Visa. Where the e-Visa is not available for a nationality, the application goes through the Indian High Commission or Embassy.
Hospital acceptance and fit-to-fly
The receiving hospital in India issues an acceptance letter, and the treating doctor issues fit-to-fly clearance. We send the case file to the hospital and confirm the bed before departure.

Three ways to travel to India
Air ambulance services between Africa and India run in three ways. The patient’s condition decides which of them are possible. The time available and the price decide between them.
Private ICU air ambulance jet
For critical or unstable patients. A jet with a full ICU team flies to the patient’s schedule. Longer routes from West and Southern Africa usually need a fuel stop.
Commercial airline stretcher
For stable patients who must lie flat. A stretcher is fitted in the airline cabin and a doctor or nurse travels alongside. The airline must give medical clearance first.
Medical escort in an airline seat
For stable patients who can sit upright. A doctor or nurse travels in the next seat, with oxygen and medicines.

Medevac and air ambulance services within Africa
Africa combines world-class hospitals with remote mining sites, safari destinations and offshore operations. Many places have limited ground infrastructure and long distances to tertiary care. We coordinate time-sensitive air ambulance and medical evacuation flights with treating hospitals, medical teams, insurers, assistance companies and families.
When an airlift is needed
- Road traffic collisions in major cities such as Lagos, Nairobi, Johannesburg and Cairo.
- Safari and adventure injuries in remote areas.
- Stroke, heart attack and sepsis cases that need ICU-level care in flight.
- Offshore and mining incidents where airlift is the only safe option.
Regional hubs
- North Africa: Cairo, Egypt.
- West Africa: Lagos, Nigeria, and Accra, Ghana.
- East Africa: Nairobi, Kenya, and Addis Ababa, Ethiopia.
- Southern Africa: Johannesburg and Cape Town, South Africa.
Central Africa is covered from these hubs. Every case is reviewed by our operations and medical coordination team, which looks at clinical safety, route planning, permits, landing clearances and the readiness of the receiving hospital.
Typical missions
- A patient travels from Africa to India for planned treatment, with a medical escort or on a stretcher.
- Evacuation from a remote airstrip to a tertiary centre in Johannesburg, Nairobi or Cape Town.
- Repatriation from Africa to India, Europe, the Middle East or North America.
- A transfer between regional centres, for example from West Africa to South Africa.
- A medical escort flight for a stable patient travelling home for recovery or rehabilitation.
Aircraft and medical equipment
For air ambulance services in Africa, we arrange bedside-to-bedside transfers through a network of experienced operators and medical providers. The aircraft is chosen for the patient’s condition, the distance, the runway and the weather.
Photos show examples of each type. The aircraft for a transfer depends on the route and the operator available. Maximum range is the manufacturer’s figure; with a patient, a medical team and fuel reserves on board the practical range is shorter. Long-range jets and full figures: air ambulance aircraft.
On an ICU flight
- ICU-configured fixed-wing aircraft for critical care patients.
- Ventilator, monitoring, infusion pumps and oxygen as required.
- Doctor and nurse or paramedic escort, depending on the severity of the case.
- Neonatal and paediatric air ambulance, subject to specialist availability.

Air ambulance services Africa: common questions
How much do air ambulance services from Africa to India cost?
We do not publish fixed rates for Africa, because the routing, fuel stops and permits differ from case to case. For scale, a private ICU jet costs about ₹27 lakh to ₹45 lakh between India and the UAE, and about ₹52.5 lakh to ₹97.5 lakh or more between India and Europe. A stretcher or a medical escort on a commercial flight costs much less. Send the medical report for a written quote.
Can a patient fly from Africa to India on a commercial flight?
Yes, if the patient is stable. Nonstop flights link Nairobi with Mumbai, Addis Ababa with Delhi and Cairo with Delhi, and other cities connect through these hubs or the Gulf. A patient who can sit upright travels with a medical escort, and a patient who must lie flat travels on an airline stretcher. The airline’s medical department must clear the patient first.
Do travellers from Africa need a yellow fever certificate to enter India?
India asks travellers arriving from countries with yellow fever risk, including Nigeria, Ghana, Kenya and Ethiopia, for a valid yellow fever vaccination certificate. The certificate becomes valid 10 days after vaccination. A traveller without one can be quarantined for up to 6 days on arrival, so tell us early if the patient or a companion has no certificate.
Which African countries and regions does Safe Fly Aviation cover?
Safe Fly Aviation coordinates air ambulance and medevac flights across North, West, East, Central and Southern Africa, through hubs including Johannesburg, Nairobi, Lagos, Accra, Addis Ababa and Cairo.
What is included in an ICU air ambulance flight in Africa?
An ICU-configured fixed-wing aircraft carries a ventilator, monitoring equipment, infusion pumps and oxygen as required, with a doctor and a nurse or paramedic depending on the severity of the case. Neonatal and paediatric air ambulance support is available subject to specialist availability.
Can patients be repatriated from Africa to India, Europe, the Middle East or North America?
Yes. Long-range medical repatriation is arranged from Africa to India, Europe, the Middle East or North America, depending on the patient’s home country or the specialist hospital required.
Do you arrange medical transfers within Africa, not just international repatriation?
Yes. Transfers between regional centres are common, for example from West Africa to a tertiary hospital in South Africa, as are evacuations from remote airstrips to centres such as Johannesburg, Nairobi or Cape Town.
Does Safe Fly Aviation operate the air ambulance aircraft in Africa?
No. In Africa, Safe Fly Aviation works with reputable, licensed air ambulance and medevac operators and does not operate the aircraft itself. Families, insurers and case managers get one point of contact for the whole transfer.
