Visas and documents

  • Indian passport holders and OCI card holders do not need a visa to enter India.
  • Other nationals travelling for treatment need an Indian Medical Visa, with Medical Attendant visas for companions. Check eligibility for the e-Medical Visa on India’s official e-Visa portal.
  • A patient or escort who connects in the United States needs a US visa or ESTA, even when only changing planes.
  • The receiving hospital in India issues an acceptance letter, and the treating doctor issues fit-to-fly clearance.

Three ways to travel to India

An air ambulance from the USA to India runs in one of three ways. The patient’s condition decides which of them are possible. The time available and the price decide between them.

Private ICU air ambulance jet

For critical or unstable patients. A long-range jet with a full ICU team flies to the patient’s schedule, usually with one or two fuel stops between the Americas and India.

Commercial airline stretcher

For stable patients who must lie flat. A stretcher is fitted in the airline cabin and a doctor or nurse travels alongside. The airline must give medical clearance first.

Business-class medical escort

For stable patients who can sit upright. A doctor or nurse travels in the next seat with oxygen and medicines. On a 15-hour flight, a lie-flat seat is usually the better choice.

Air ambulance services in South America, Central America and the Caribbean

The Americas cover dense cities, island destinations, mountain ranges, rainforest and offshore operations. Patients may need to be moved from a remote location to a regional hub, or from one country to another, for specialist care. Our role is to bridge distance, language and regulatory differences, and to give families and case managers one point of contact for the whole transfer.

What we help manage

  • Hospital-to-hospital transfers in North America.
  • Evacuations from Latin America and the Caribbean after an accident or sudden illness.
  • Transfers from South America to North America or Europe for advanced treatment.
  • Repatriation to home countries in Asia, Europe, Africa or the Middle East.

Hubs

  • North America: New York, Miami and Houston in the United States; Toronto in Canada.
  • South America: São Paulo and Rio de Janeiro in Brazil; Bogotá in Colombia; Santiago in Chile.
  • Central America: Mexico City.
  • Caribbean: Nassau in The Bahamas; San Juan in Puerto Rico; other island gateways on request.

Every case is reviewed by our operations and medical coordination team, which looks at clinical safety, route planning, permits and the readiness of the receiving hospital.

Typical scenarios

  • An Indian parent falls ill while visiting family in the USA or Canada and needs to return to India with medical support.
  • A tourist is evacuated from a Caribbean island to a major hospital in Florida or to the home country.
  • A patient is transferred from South America to North America or Europe for advanced treatment.
  • A staff member is injured on an offshore platform and needs specialist surgery.
  • An overseas citizen is repatriated after long-term treatment in the Americas.

Distances and aircraft for a private air ambulance

The Americas are the longest air ambulance routes to India. Even an ultra-long-range jet usually stops for fuel with a patient and a medical team on board, and from South America two or more stops are normal.

Great-circle distances between airports, rounded. The routing for each flight depends on the aircraft, the winds and the permits.

Air ambulance USA to India and the Americas: common questions

How much does an air ambulance from the USA to India cost?

Between the USA and India, a transfer costs about ₹18.75 lakh to ₹37.5 lakh with a medical escort on a commercial flight, ₹25 lakh to ₹50 lakh on a commercial stretcher, and ₹1.05 crore to ₹2.25 crore or more by private ICU jet. These are budget ranges reviewed in October 2026, and they apply in either direction. Each transfer is quoted in writing after the medical report is reviewed.

Can a patient fly from the USA or Canada to India on a commercial flight?

Yes, if the patient is stable. A patient who can sit upright can travel in business class with a doctor or nurse, and a patient who must lie flat can travel on an airline stretcher. The airline’s medical department must clear the patient first, which usually takes a few days. Critical or unstable patients need a private ICU air ambulance.

How long does a medical flight from the USA or Canada to India take?

Nonstop scheduled flights from New York or Toronto to Delhi take about 14.5 to 16 hours. A private air ambulance jet usually takes longer, because most need one or two fuel stops on this distance. Allow extra time for airline medical clearance, visas and flight permits before departure.

Which airlines fly nonstop between North America and India?

At the time of writing, Air India, American Airlines and United fly nonstop between the New York area and Delhi, and Air Canada and Air India fly nonstop between Toronto and Delhi. Air India also serves Vancouver and San Francisco, and adds Mumbai to Toronto from 25 October 2026. Schedules changed several times in 2026, so we check the flights operating on the day.

Do you arrange air ambulance services in South America and the Caribbean?

Yes. Safe Fly Aviation works with established air ambulance and medevac operators across Central and South America and the Caribbean, through hubs including São Paulo, Rio de Janeiro, Mexico City, Bogotá, Santiago, Nassau and San Juan. Patients can be moved to a regional hub, to North America or Europe, or home to India.

Can patients be repatriated from the Americas to Europe, Asia, Africa or the Middle East?

Yes. As well as hospital-to-hospital transfers in North America and evacuations from Latin America and the Caribbean, we arrange repatriation flights to a patient’s home country in Europe, Asia, Africa or the Middle East.

What affects the timeline for an air ambulance flight from the Americas?

Visa requirements, immigration processes, medical documents and insurance approvals all affect the timeline and the route, along with the patient’s clinical stability, the aircraft available, airport options and the weather.

Does Safe Fly Aviation operate the air ambulance aircraft in the Americas?

No. In the Americas, Safe Fly Aviation works with established, reputable air ambulance and medevac operators and does not operate the aircraft itself. Families and case managers get one point of contact for the whole international transfer.
