Medical repatriation between Australia and India: who we move

About 971,000 people born in India lived in Australia at 30 June 2025, which made India the largest overseas country of birth for the first time, according to the Australian Bureau of Statistics. Families are spread across both countries, so patients travel in both directions.

  • Patients leaving hospital who cannot manage a 12-hour flight without medical support.

  • Patients travelling to Australia for planned treatment.
  • Patients moving closer to family in Australia after a hospital stay in India.

Inside Australia, emergency retrieval from regional and remote areas is the work of state ambulance services and the Royal Flying Doctor Service. In an emergency in Australia, call 000. Our part is the international transfer and the link to the departure airport.

Before you fly

Four things can delay a transfer between Australia and India. Start them as soon as the transfer is being considered.

  • Airline medical clearance

A stretcher or an escorted seat needs the airline’s medical clearance first. Qantas asks for its medical travel clearance form at least 7 days before travel. Air India uses the MEDIF form, with a separate form for each sector.

  • Visa for India

Indian passport holders and OCI card holders do not need a visa. Australian citizens can apply online for an e-Medical Visa at least 4 days before arrival, with up to two e-Medical Attendant visas for companions.

  • Visa for Australia

A patient travelling to Australia for treatment applies for the Medical Treatment visa (subclass 602), which also covers a support person. Australian citizens returning home do not need a visa.

  • Insurance and hospital bills

The Australian Government does not pay medical or evacuation costs for travellers overseas, and India has no reciprocal health care agreement with Australia. Ask the insurer early whether the policy covers repatriation, and settle the discharge with the hospital.

Visa and airline rules change, so check the official pages before you apply. More on visas and hospitals in India: medical tourism in India.

Three ways to travel

Air ambulance services between Australia and India run in three ways. The patient’s condition decides which of them are possible. The time available and the price decide between them.

Private ICU air ambulance jet

For critical or unstable patients. A jet with a full ICU team flies to the patient’s schedule. This is one of the longest air ambulance routes: a long-range jet usually makes one fuel stop, often in Singapore, and smaller jets need more.

Commercial airline stretcher

For stable patients who must lie flat. On Qantas a stretcher takes the space of eight economy seats, an international booking is made through a repatriation company, and a doctor and a nurse must both travel. Air India carries one stretcher per flight, in economy, on its own flights.

Medical escort in an airline seat

For stable patients who can sit upright. A doctor or nurse travels in the next seat, with oxygen arranged through the airline where it is needed. On a flight this long, a lie-flat business-class seat is usually the better choice.

Distances and aircraft for a private air ambulance

For air ambulance services from Australia, distance decides the aircraft. Only an ultra-long-range jet can cover the east coast to Delhi without a stop, so most flights are planned around one fuel stop.

Great-circle distances between airports, rounded. The routing for each flight depends on the aircraft, the winds and the permits.
RouteDistanceTypical private jet routing
Sydney to Delhi10,400 kmOne fuel stop on a long-range jet, for example Singapore: 6,300 km, then 4,150 km.
Melbourne to Delhi10,200 kmOne fuel stop on a long-range jet, for example Singapore.
Brisbane to Delhi10,200 kmOne fuel stop on a long-range jet, for example Singapore.
Adelaide to Delhi9,550 kmOne fuel stop on most jets.
Perth to Delhi7,900 kmThe shortest crossing from Australia. One fuel stop on most jets.
Sydney to Bengaluru9,350 kmOne fuel stop on most jets.

Photos show examples of each type. The aircraft for a transfer depends on the route and the operator available. Maximum range is the manufacturer’s figure; with a patient, a medical team and fuel reserves on board the practical range is shorter. Full figures: air ambulance aircraft.

How a transfer between Australia and India works

  1. Medical report and fit-to-fly

Send the medical report. Our medical team reviews it with the treating doctor, who confirms whether the patient is fit to fly and what support is needed in the air.

  2. Options and a written quote

We set out which of the three ways to travel are medically suitable, with a written quote for each.

  3. Clearances and visas

We obtain the airline’s medical clearance or the flight permits, and tell you which visas the patient and companions need.

  4. Bed-to-bed transfer

A ground ambulance takes the patient from the hospital bed to the aircraft, the medical team stays with the patient throughout, and a second ambulance completes the handover at the receiving hospital.

The air ambulance booking guide lists the documents needed at each stage.

Air ambulance services Australia: common questions

How much do air ambulance services from Australia to India cost?

Australia is not yet on our published rate list, so every transfer is quoted in writing after the medical report is reviewed. For scale, a private ICU jet costs about ₹60 lakh to ₹1.05 crore or more between India and the United Kingdom, a distance of about 6,700 km, and about ₹1.05 crore to ₹2.25 crore or more between India and the United States. Sydney is about 10,400 km from Delhi. A stretcher or a medical escort on a commercial flight costs much less than a private jet.

Can a patient fly from Australia to India on a commercial flight?

Yes, if the patient is stable. Nonstop flights link Delhi with Sydney and Melbourne, and Sydney with Bengaluru, and other cities connect through these or through Singapore. A patient who can sit upright travels with a medical escort, and a patient who must lie flat travels on an airline stretcher. The airline’s medical department must clear the patient first; Qantas asks for its form at least 7 days before travel.

How long does a medical flight from Australia to India take?

Nonstop scheduled flights take about 12 to 13 hours between Delhi and Sydney or Melbourne, and about 11 hours 30 minutes between Sydney and Bengaluru. A private air ambulance usually takes longer, because it stops for fuel on the way. Allow several days before departure for airline medical clearance, visas and flight permits.

Which Australian cities does Safe Fly Aviation cover?

Safe Fly Aviation arranges transfers from Sydney, Melbourne, Brisbane, Perth and Adelaide. Patients in Darwin, Canberra, Hobart, regional centres and New Zealand first travel to a gateway city by road ambulance, domestic flight or air ambulance.

Does the Australian Government or insurance pay for medical repatriation to India?

The Australian Government does not pay medical or evacuation costs for travellers overseas, and India has no reciprocal health care agreement with Australia. Whether a travel, visitor or student health policy pays for repatriation depends on the policy, so ask the insurer early. We provide a written quote and the case documents an insurer asks for.

What visa does the patient need?

Indian passport holders and OCI card holders do not need a visa for India. Australian citizens can apply online for an Indian e-Medical Visa at least 4 days before arrival, with up to two e-Medical Attendant visas for companions. A patient travelling to Australia for treatment applies for the Medical Treatment visa (subclass 602).

Can you bring a patient from India to Australia?

Yes. The same three ways to travel apply in the other direction: a private ICU jet, an airline stretcher or a medical escort. The receiving hospital or doctor in Australia must agree to take the patient before departure.

Does Safe Fly Aviation operate the aircraft on Australia routes?

No. Flights are flown by licensed air ambulance operators and scheduled airlines. Safe Fly Aviation arranges the transfer and gives families, insurers and case managers one point of contact from the first call to the hospital handover.
