Commercial Airline Stretcher Service to and from India
A stable patient who must lie flat can fly on a scheduled airline, on a stretcher fitted in the cabin with a medical escort alongside. It costs far less than a private air ambulance.
A commercial airline stretcher service flies a stable patient lying flat on a scheduled flight. The airline fits a stretcher over a block of economy seats, and a medical escort travels beside the patient. Within India it costs about ₹1.9 lakh to ₹5.95 lakh, against ₹4.5 lakh to ₹22.5 lakh for a private ICU aircraft. The airline must give medical clearance first, usually at least 48 to 72 hours before departure.
How an airline stretcher works
The patient travels on a normal scheduled flight. The airline prepares the cabin, and we arrange the medical escort, the paperwork and the ambulances.
A stretcher over economy seats
The airline folds down a block of economy seats and fixes a stretcher above them. How many seats depends on the airline and the aircraft. Qantas, for example, blocks eight.
A curtain for privacy
A curtain is usually fitted around the stretcher, as in the Airbus A320 photo above, so the patient has privacy during the flight.
A medical escort throughout
A doctor, nurse or paramedic travels beside the patient with a portable monitor, suction and medicines. Cabin crew do not give medical care, so airlines require an escort.
Oxygen arranged in advance
In-flight oxygen is requested from the airline with the medical form. Any other equipment the patient needs has to be approved by the airline before the flight.
Ambulances at both ends
A ground ambulance brings the patient to the aircraft and another meets the flight. Airlines expect the patient’s side to arrange both, so we book them with the flight.
Medical clearance first
The treating doctor completes a Medical Information Form (MEDIF). The airline’s medical desk reviews it and decides whether the patient can fly.
How we arrange a stretcher transfer
Send the medical summary
We go through it with the treating doctor and confirm that a stretcher on an airline is the right way to move the patient.
We choose the airline and flight
A direct flight where there is one. Some airlines carry stretchers only on non-stop flights, or only on aircraft they operate themselves.
The airline gives medical clearance
We submit the MEDIF and the medical report. Emirates and Qatar Airways ask for it at least 48 hours before departure, Etihad at least 72 hours.
Travel day
An ambulance takes the patient to the airport. The stretcher is already fitted, and the medical escort stays beside the patient for the whole flight.
Handover at the destination
An ambulance meets the aircraft. The escort hands the patient and the medical notes to the receiving hospital or the family.
Who can travel on an airline stretcher
The treating doctor and our medical team advise, and the airline’s medical desk makes the final decision.

Needs a private air ambulance instead
- Unstable, or likely to need intensive care during the flight
- Has to move today or tomorrow
- Has an infection the airline will not accept in the cabin
- Has been refused clearance by the airline’s medical desk
Stretcher, medical escort or private air ambulance
Three ways to move a patient by air. The patient’s condition decides which are possible; the price and the time available decide between them.

Not every airline, aircraft or route can take a stretcher. We check with the airline for the route you need before quoting.
Consider Bring You Home instead
A patient who is stable and can sit does not need a stretcher. A doctor or critical-care nurse escorts them in a business-class seat, which is often more comfortable and costs less, from about ₹1.55 lakh within India.

Commercial stretcher flights: common questions
What is a commercial airline stretcher service?
It is a way to fly a patient who must lie flat on a scheduled airline flight instead of a private aircraft. The airline fits a stretcher over a block of economy seats, usually with a curtain around it, and a medical escort travels beside the patient. It suits stable patients and needs the airline’s medical clearance in advance.
How much does a stretcher on a commercial flight cost?
Within India a commercial stretcher transfer costs about ₹1.9 lakh to ₹5.95 lakh, depending on the route. Between India and the United Arab Emirates it costs about ₹7.5 lakh to ₹15 lakh. On the routes in our price list that is roughly 60 to 80 per cent less than a private ICU aircraft. These are indicative ranges reviewed in October 2026; every transfer is quoted in writing before you confirm.
Which airlines allow stretchers on board?
Air India, Emirates, Qatar Airways, Etihad Airways, Lufthansa and Qantas all publish rules for carrying a passenger on a stretcher. Each asks for medical clearance in advance and for an escort to care for the patient. The rules differ between airlines and change over time, and the airline decides case by case whether to accept a patient.
Does Air India carry stretcher patients?
Yes. Air India accepts stretcher passengers in economy class on flights it operates itself, with one stretcher passenger per flight. Bookings are one-way and are made at Air India offices, an adult escort must travel with the patient, and a Medical Information Form (MEDIF) is needed for each flight sector. This is Air India’s published policy, checked on 10 October 2026.
How many seats does an airline stretcher take?
The stretcher is fixed over a block of economy seats that the airline takes out of sale, and the fare reflects those seats. The number depends on the airline and the aircraft. Qantas, for example, blocks eight economy seats and charges eight economy fares.
How far ahead must a stretcher flight be booked?
A stretcher cannot be arranged on the day. Emirates and Qatar Airways ask for the medical form at least 48 hours before departure, Etihad at least 72 hours, and Qantas at least 7 days. The seats also have to be free on the flight. A patient who cannot wait should travel by private air ambulance.
Can a critically ill patient travel on an airline stretcher?
Airline stretchers are mainly for stable patients. Some airlines accept more dependent patients when a full medical team and the equipment are arranged in advance, and Lufthansa offers an enclosed Patient Transport Compartment for intensive care on specific long-haul aircraft. An unstable or time-critical patient should travel by private air ambulance. The treating doctor, our medical team and the airline’s medical desk decide together.
What is the difference between a stretcher flight and a medical escort flight?
On a stretcher flight the patient lies flat on a stretcher fitted in the cabin. On a medical escort flight the patient sits in a normal airline seat, often in business class, with a doctor or nurse alongside. An escort flight costs less, about ₹1.55 lakh to ₹4 lakh within India, and suits a patient who can sit upright for take-off and landing.
